• Proof of Delivery (POD)
• Bill of Lading (BOL)
• Original Cargo/Liability Insurance naming MTS Transportation, Inc. as Certificate Holder
• Signed MTS Contract
• Taxpayer Certificate (Form W-9)
• ICC Authority
• Frozen Load Agreement (Refrigerated loads only)
